What is AI Incident Response?

AI incident response is the process of detecting, investigating, and remediating incidents involving AI usage: data exposure, policy violations, harmful outputs, runaway costs, or model misuse. It extends security incident response to a domain most playbooks do not yet cover.

The defining challenge is evidence. Investigating an AI incident requires reconstructing the interaction — who, what prompt, which model, what response — and most organizations have no record to consult.

Preparation therefore matters more than reaction: logging at the checkpoint, retention policies, and the ability to search and replay interactions turn an unanswerable incident into a manageable one.
